Our ultimate goal at Project Six/The Commons is to prepare youth to transition to a less restrictive environment and to be able to participate in their communities to their fullest potential. The Commons offered several transitional services to support youth as they prepare to discharge from residential treatment. Transitional Services may include:
- Obtaining CA identification
- Obtaining SSI
- Participating in supervised work experience
- Exploring possible Regional Center services
- Exploring housing options for youth preparing for independent living
- Exploring conservatorships and supporting parents through this process
- Providing referrals for post-discharge mental health and community-based services
